The SNFist team partnered with a Primary Care practice in Georgia that mainly services the post-acute market. In the early stages of this partnership, the implementation of SNFist strategy and systems were executed with little, to no, disruption of current practices. This allowed the practice early gains, that are being reinvested into scaling the practice. Also within the first year SNFist was able to successfully complete a joint venture of a nearby like-minded, forward-thinking practice. We look to further this partnership over the coming years by applying SNFist’s fundamental principles that were agreed upon during our planning sessions with the practice’s leadership.
